Elementary Transfer Request Process, grades K-6

Complete the form above, make copies of two proofs of residency, and mail or bring those three documents to: Kari Leier, Hughes Educational Center, 806 N. Washington Street, Bismarck, ND 58501, Monday-Friday 8 am to 4 pm, 701-323-4110.
Two proofs of residency: (must be provided by the legal parent/guardian living in the Bismarck School District) (one primary proof of residency and one secondary proof of residency must be submitted with transfer request).
One primary proof of residency: home mortgage, builder's agreement, deed, OR a lease/rental agreement that lists the names of the parents/guardians living in the rental unit, plus the manager's name and phone number.
One secondary proof of residence: a bill for heat/lights, garbage/water, or cable TV dated within the last 30 days, OR a document from the Department of Social Services.
Unacceptable proof of residence:- Post office change of address
- Credit card/bank statements
- Personal taxes
- Medical bills
- Payroll checks
- Auto insurance policy
- Any proof older than 30 days
Secondary Transfer Request Process, grades 7-12
To request a transfer for students entering grades 7-12, please call Kari at the Central Registration Office, 701-323-4110.
Secondary Transfer Request forms are unavailable online and must be filled out at the Central Registration Office by the legal parent/guardian.
